Information Needed to Provide a Benefits Quote

The following information details what is required to provide a customized, integrated benefits quote for pre and/or post-65 retirees.

As you would expect, while all the information that we are requesting is required, certain pieces of information are more critical than others. We have specified below, the information that we must have, by priority, to proceed with obtaining a quote.

Census data is the first component of building a quote model.

Data fields in the census data for Pre-65 and Post-65 Retirees

  • Name (required)
  • Social Security number or other type of identification (optional)
  • Address with zip code (required)
  • Birth date (required)
  • Gender (required)
  • Marital Status (required)
  • Level of coverage (single,family, etc) (required)
  • Retiree contribution (required)

Data that is necessary for Pre-65 and Post-65 Retirees

  • Three Years Aggregate Claims Experience
  • Three Years Enrollment
  • Three Years Monthly Claims Experience
  • Three Years Large Claims Experience
  • Summary of Plan(s) Benefits and desired changes to current benefits
  • Description of Early Retiree Policy (for pre-65 retiree quote only)

Information that will expedite the quoting process

  • Summary Plan Descriptions (SPDs)
  • Definition of an eligible employee/dependent/retiree
  • Section 125 Plan Design
  • Copies of the prior year's communications for open enrollment
  • Description of collective bargaining agreements
